It is an irreversible beta-lactamase inhibitor for parenteral use only. Chemically it is sodium penicillinate sulfone. It contains 92 mg sodium (4 mEq) per gram. Sulbactam is an off-white crystalline powder which is highly soluble in water. The molecular weight is 255.22. Cefoperazone sodium I.P. is a semisynthetic broad-spectrum cephalosporin antibiotic for parenteral use only. It contains 34 mg sodium (1.5 mEq) per gram. Cefoperazone is a white crystalline powder which is freely soluble in water. The molecular weight is 667.65. In addition to the active ingredient, anidulafungin, DULATECT for Injection contains the following inactive ingredients: fructose (50 mg), mannitol (250 mg), polysorbate 80 (125 mg), tartaric acid (5.6 mg), and sodium hydroxide and/or hydrochloric acid for pH adjustment.\n\nCLINICAL PHARMACOLOGY Pharmacokinetics The pharmacokinetics of anidulafungin following IV administration have been characterized in healthy subjects, special populations and patients. Systemic exposures of anidulafungin are doseproportional and have low intersubject variability \n\nThe pharmacokinetics of anidulafungin following IV administration are characterized by a short distribution half-life (0.5-1 hour) and a volume of distribution of 30-50 L that is similar to total body fluid volume. Anidulafungin is moderately bound to plasma proteins in humans (84%).\n\nExcretion In a single-dose clinical study, radiolabeled (14C) anidulafungin was administered to healthy subjects. Approximately 30% of the administered radioactive dose was eliminated in the feces over 9 days, of which less than 10% was intact drug. Less than 1% of the administered radioactive dose was excreted in the urine. Anidulafungin concentrations fell below the lower limits of quantitation 6 days post-dose. Negligible amounts of drug-derived radioactivity were recovered in blood, urine, and feces 8 weeks post-dose.\n\nMICROBIOLOGY \n\nMechanism of action Anidulafungin is a semi-synthetic echinocandin with antifungal activity. Anidulafungin inhibits glucan synthase, an enzyme present in fungal, but not mammalian cells. Teicoplanin inhibits the growth of susceptible organisms by interfering with cell wall biosynthesis at a site\ndifferent from that affected by â-lactams. It is active against staphylococci (including those resistant to methicillin and other â-\nlactam antibiotics), streptococci, enterococci, Listeria monocytogenes, corynebacteria and gram-positive anaerobes including\nClostridium difficile and peptococci.\nBactericidal synergy has been demonstrated in vitro with teicoplanin when combined with aminoglycosides against\nStaphylococcus aureus and enterococci; synergism has also been demonstrated with imipenem against these organisms. The\nin vitro combination of teicoplanin and rifampin has shown additive and synergistic effects against Staphylococcus aureus. In\nvitro synergy with ciprofloxacin against Staphylococcus epidermidis has also been observed. Teicoplanin does not show crossresistance\nwith other classes of antibiotics. Some cross-resistance is observed between teicoplanin and the glycopeptide\nvancomycin among enterococci. Teicoplanin is taken up by leukocytes and macrophages, and retains antistaphylococcal\nactivity within these cells.\nPharmacokinetics: Teicoplanin is administered by parenteral injection. The bioavailability of a single 3-6 mg/kg IM injection is\n>90%. Following oral administration, teicoplanin is not systemically absorbed from the normal gastrointestinal tract; 40% of\nthe administered dose is present in the feces in a microbiologically active form.\nFollowing IV administration of 3-6 mg/kg, the plasma concentration declines with a terminal elimination half-life of about 150\nhrs; total plasma clearance ranges from 11.9-14.7 mL/hr/kg. This long half-life allows once a day administration. At 6 mg/kg\nadministered IV at 0, 12, 24 hrs and every 24 hrs thereafter as 30-min infusion, a predicted trough serum concentration of 10\nmg/L would be reached by day 4. Predicted steady-state peak and trough serum concentrations of approximately 64 and 19\nmg/L, respectively, would be attained by day 28.\nTeicoplanin distributes readily into the skin (subcutaneous fat) and blister fluid, myocardium, pulmonary tissue and pleural\nfluid, bone and synovial fluid but not readily into cerebrospinal fluid (CSF). The main biological functions of lipids include: storing energy, signaling, and acting as structural components of cell membranes. \nPhospholipids are a class of lipids that are a major component of all cell membranes as they can form lipid bilayers. The structure of the phospholipid molecule generally consists of hydrophobic tails and a hydrophilic head. \nA liposome is a spherical vesicle that has at least one lipid bilayer and an aqueous core. The liposome can be used as a vehicle for administration of nutrients and pharmaceutical drugs\nBenefits and Applications\n➢Due to their unique properties, including low cytotoxicity, good biocompatibility and biodegradability, liposomes have wide-ranging applications in different fields including gene and drug delivery, food and nutrition industries and cosmetic industries. \n➢A number of new liposome modification methods have emerged to improve stability and attain higher concentrations of bioactives in the target cells and cellular compartments for maximum therapeutic efficiency. \n➢Active targeting can be achieved via appropriately engineered modifications to the liposomal structure. For active targeting, thermo-labile, pH-sensitive, photo-sensitive and antibody coated vesicles, have been designed.\nPassive targeting is the mechanism by which the bioactive-carrier complex reaches its destination based on the physicochemical properties of bioactive carrier complexes and does not utilize any targeting strategy\nMechanism\n➢A liposome has an aqueous core enveloped by a phospholipid bilayer.\n➢Hydrophilic solutes dissolved in the core cannot readily pass through the bilayer. Hydrophobic chemicals associate with the bilayer via Van der Waals forces.\n➢This makes liposomes versatile and advantageous delivery vehicles as they can be formulated with hydrophobic and/or and hydrophilic cargo. To deliver the molecules to a site of action, the lipid bilayer can fuse with other bilayers such as the cell membrane, thus delivering the liposome contents.\n➢Useful liposomes rarely form spontaneously. They typically develop after supplying enough energy, in the form of sonication, to a dispersion of phospholipids in a polar solvent, such as water, to break down multilamellar aggregates into oligo- or unilamellar bilayer vesicles.\n➢Mechanism of Delivery\n➢By preparing liposomes in a solution of DNA or drugs (which would normally be unable to diffuse through the membrane) they can be (indiscriminately) delivered past the lipid bilayer, but are then typically distributed non-homogeneously.\n➢For drug delivery, liposomes that contain low (or high) pH can be constructed such that dissolved aqueous drugs will be charged in solution (i.e., the pH is outside the drug's pH range).\n➢As the pH naturally neutralizes within the liposome (protons can pass through some membranes), the drug will also be neutralized, allowing it to freely pass through a membrane. These liposome's work to deliver drug by diffusion rather than by direct cell fusion.",
